Protection of Personal Information

Our office policy is to protect and safeguard the confidential nature of personal, non-public information that may be obtained concerning our employees, patients and other individuals.  This information includes but is not limited to social security numbers, driver’s license numbers, state identification cards, account numbers, credit or debit card numbers, passport numbers, alien registration numbers and health insurance identification numbers. (This information is referred to hereafter as “Personal Information”).


Our office will disclose Personal Information on a strict business need-to-know basis and to the extent required or permitted by law.  We will use all reasonable safeguards to prevent unauthorized access and disclosure of Personal Information.  Although security cannot be totally guaranteed, Dr. Salonia will maintain physical, electronic and procedural safeguards to minimize the risk of unauthorized access of such information. 


Our office shreds on site any patient and business records that contain Personal Information prior to disposing of such information.  We dispose of Personal Information by contracting with a firm formally engaged and approved in the business of disposing of records that contain confidential information.


Our employees are prohibited from disclosing or revealing Personal Information for unauthorized purposes.  Employees of this office may only acquire and use Personal Information for legitimate business purposes within this office and must safeguard the privacy of the information and take reasonable measures to ensure that said information is protected from disclosure or misappropriation.  Consistent with these policies, Dr. Salonia may impose disciplinary measures for actions not in compliance with this policy.  All complaints or allegations of violations of this policy will be investigated by Dr. Salonia and appropriate action will immediately be taken.
